Eyelash extension consent form maker
Add your business, choose your services, and print a client intake and consent form, or copy the text into your booking app.
A template, not legal advice. Rules on deposits, fees and client forms vary by state; check with your state board or a local professional if you're unsure.
What the form includes
Client details, the services she's booked, health questions that matter for lash work (allergies, eye conditions, recent eye surgery, contact lenses, pregnancy), what she understands about the risks and aftercare, an optional patch-test record, an optional photo permission, and signature lines for both of you.
How to use it
Print it and have each new client fill it in before her first appointment, or copy the text into your booking app's intake form so she signs it online. Keep signed forms for as long as your state requires client records to be kept, and review the answers before every service, not only the first.
Choose "Print or save as PDF" in the tool: on most phones and computers the print screen can save a PDF instead of printing.
This is a template, not legal advice
What a consent form must say, and how long you must keep it, differs by state and by the services you offer. Ask your state's cosmetology board or a local professional to check it before you rely on it. The same note is printed at the bottom of the form.
Questions
Do I need a consent form for lash extensions?
Most lash artists use one, and many insurers and training programs expect it. What's required by law differs by state, so check with your state's cosmetology board.
Can I use this form for lash lifts and tints?
Yes. Choose lash lift or tint in the services and the form adds the questions and statements that go with them.
Is the form stored anywhere?
No. It's made in your browser and printed or copied from there. Nothing is sent to WIPE.
